Instructions for filling out and saving your completed PDF application. 

1.    Download and open the form: Save the blank PDF to your computer. Right-click the file and ensure it's set to open with your PDF application, such as Adobe Acrobat Reader

2.    Fill out the form: Open the file and enter your information into the fillable fields. 

3.    Save the form: Go to File > Save or File > Save As to save the completed document. Saving with "Save As" lets you choose a new file name and location to distinguish it from the original blank form.

DO NOT convert your completed form to any other format or file extension. The file you return must remain the file type: .pdf. NO OTHER FILE EXTENSION will be accepted. Finally, email the completed form as an attachment using your preferred email client.
