Forms and Documentation
All NYS County Boards and Divisions are asked to download and return or fill out and submit online the necessary forms (found below) each year so that they are completing and filing the most up-to-date version with the proper jurisdiction. If you have any questions regarding which form to file, please contact the appropriate officer.

New York State Certificate of Election. Due annually on February 15.

Application for charter. Submit as necessary to the National Secretary

New York State Division Financial and Membership Report, Due annually February 15
Form 28
Application to form a division. Submit as necessary to the National Secretary

State and County Board Financial Report for Tax-Exempt Status Due annually on February 15.

Required to be submitted every year.
